To apply for an Adjustable Rate Mortgage Loan, follow these directions:

| 1. Click on ARM disclosure below. Print out two copies of the ARM Disclosure. Keep one for your records. Sign and date the other and submit with your application. |
| 2. At the bottom of the ARM disclosure page, click on "Go To Application." Complete the Residential Loan Application on your computer. Print out and sign the application. |
| 3. Mail, fax or drop off the Application and ARM Disclosure. |
